Provide a few details
Select “Apply Now” and fill out a quick digital intake form. Have your State ID, Proof of Insurance (if applicable), and credit or debit card ready for payment

appointment time
Our office hours are M-F 10a-6p. Select the time that works best for you to connect with our licensed physicians

Join your digital appointment
10 minutes before your appointment, you’ll receive a phone call from our office. At this time, we will go over your application and connect you with one of our licensed physicians. Have your photo ID ready!

WHAT TO EXPECT AFTER
YOUR APPOINTMENT

A Confirmation Email
After your appointment, you will receive a confirmation email from OMMA within 2-5 days. This means that your application has been submitted, and state agencies are processing your information.

Approval or Rejection Notice
10-14 days after your appointment, you should receive an approval or rejection notice from OMMA. If your license happens to be rejected, please call our offices so we can resubmit the application for you!

Your Card in the Mail
You should receive your Patient License in the mail within a week of your approval or rejection notice. If you do not receive your card in the mail, please call our office so we can connect you with an OMMA agent.
